Who Qualifies?
You may be eligible if:
• At least one household member is enrolled in NJ FamilyCare with an MCO
• You are homeless, at risk of homelessness, or need housing to stay in the community
And
You have one or more of the following
(can be self-reported):
• Chronic or complex health condition
• Mental health or substance use issue
• Pregnancy
• Victim of domestic violence or trafficking
• Intellectual or developmental disability
Services Include:
Pre-tenancy – Help finding housing
Tenancy Support – Help staying housed
Move-in Support – Help with basic move-in needs
Housing Supports Program is a set of housing services created to ensure Medicaid/NJ FamilyCare members can live in a safe, healthy, and affordable home.
